Medical Referrals
Referring patients to Reach for Health is a straightforward process for medical professionals. Although we are an independent charity and receive no NHS funding, we are proud to be the only non-NHS organisation in Northamptonshire granted an NHS.net email address for referrals by NENE Clinical Commissioning Group (CCG).
Our referral form is available on the NENE CCG portal and can also be downloaded from this page. Completed forms should be emailed to: nccg.reachforhealthreferrals@nhs.net.
Our Daventry centre provides specialist rehabilitation support for patients recovering from major health trauma, as well as those with long-term and chronic health conditions. We also accept referrals for a wide range of general health issues including:
